Job Description
Our Client is seeking a Disease Intervention Specialist 2 / RISE (Recovery, Information, Support, and Engagement) Navigator to provide screening, assessment, outreach, counseling, referral, and recovery support services within the assigned East Tennessee county.
The position will focus on connecting individuals with substance use disorder treatment, mental health services, harm reduction resources, primary healthcare, and recovery support services. The role will also involve community outreach, education, documentation, partnership development, and travel within the assigned service area.
Responsibilities
- Conduct substance use disorder (SUD) screenings using validated tools and perform evidence-based mental health and substance use disorder assessments.
- Provide crisis intervention and solution-focused counseling in and outside health department settings.
- Educate communities on drug overdose prevention and communicable diseases, including Hepatitis C and HIV.
- Conduct outreach through non-traditional community organizations to identify individuals in need of services.
- Distribute naloxone and overdose education materials to clients and the public.
- Develop and maintain partnerships with community resources, including Syringe Service Providers (SSPs), Regional Overdose Prevention Specialists (ROPs), mental health agencies, and treatment providers.
- Connect clients with community mental health, substance use treatment, harm reduction, primary health, and recovery support services.
- Complete progress notes, track referrals, and accurately maintain information in record systems.
- Maintain records in compliance with contract, state, grant, and agency requirements.
- Participate in team meetings, case conferences, and required training.
- Support continuous improvement and service delivery initiatives.
- Perform other related duties as required.
- Travel throughout the assigned service area is required; mileage reimbursement will be provided.
Qualifications
- Must be at least 18 years of age.
- High school diploma, GED, or equivalent required.
- Understanding and support of medication-assisted recovery practices.
- Ability to develop and maintain relationships with individuals and community organizations.
- Familiarity with community-based recovery support services.
- Experience providing peer support and building effective relationships.
- Strong commitment to treating all individuals with dignity and respect.
- Strong active listening skills and ability to ask supportive, open-ended questions.
- Ability to maintain appropriate professional boundaries.
- Cultural competency and experience working with diverse populations.
- Strong customer service skills.
- Proficiency with Microsoft Office Suite.
- Bilingual Spanish/English is preferred.
Special Qualifications
- Valid driver's license, personal vehicle, and proof of insurance may be required.
- Successful completion of required background checks and registry reviews is required.
